Honors Activity Agreement Forms

All Honors College students must complete an honors activity each fall and spring semester. Honors activities may be conducted during summer session.

In addition to registering for HON 222 (most activities) or HON 322 (Capstone), students must also complete an Agreement Form at the beginning of each semester and submit a printed copy to the Honors College offices.

Please select the Honors Activity you are undertaking this semester and you will be directed to the appropriate agreement form. More information about the options for Honors Activities can be found here.

For my Honors Activity this semester I am:

  • Taking an Honors College course
  • Doing a supplement to a regular course
  • Taking advanced coursework outside of my major
  • Studying abroad
  • Doing an internship, CoOp or practicum
  • Performing a service activity
  • Doing research (Undergraduate Research Assistant or Independent Study)
  • Starting my capstone this semester
  • Continuing and/or completing my Capstone this semester

Check with your fellow about other imaginative ideas for fulfilling your semester honors activity in ways that don’t fit the categories above. You can use the other activities form to declare your activity for the semester.

Honors Activity Form Instructions

A step-by-step guide to submitting all paper work associated with your Honors Activity. This process does not include information for those completing their Capstone. For Capstone information click here.

Honors Activity Agreement Form Instructions  

To be completed at the beginning of each semester for which you are seeking Honors Activity credit. You should also be registered for HON 222.  

  1. Enter the Honors Activity Reporting System (HARS). Find the link on the top of the Honors College website.
  2. Select which type of activity you are choosing to submit for credit this semester. Your options will be listed on the left hand side of the page.
  3. Complete the appropriate form with all relevant information.
    • Notes:
      1. First year students will not yet have an assigned Fellow and should leave this portion of the form blank.
      2. If you’re using coursework for your Honors Activity credit then the activity supervisor is your current instructor for that course.
  1. Select Finish to submit the form.
  2. Once you’ve submitted the Agreement form you will be redirected to a page titled “Obtain Signatures”. A link for your Agreement form will be available on this page and a copy will be sent to your UIC email as well.
  3. You must print off the Agreement Form and get it signed by your Activity Supervisor and Fellow (if one has been assigned).
  4. Once signed, the paper copy of this form with signatures must be turned in to the Honors College front desk no later than 4pm the third Friday of the semester.

Honors Activity Completion Form Instructions  

To be completed at the end of each semester where a subsequent Agreement Form was submitted.

  1. Enter the Honors Activity Reporting System (HARS). Find the link on the top of the Honors College website.
  2. On the home page you will find a table with a list of all Honors Activities you have submitted. Find the current semester activity (should have the status as Pending Completion Signature) and click once anywhere in the row.
  3. A button that says “Print Activity Completion Form” will appear below the row of the current activity. Select this button and print the Completion Form.
  4. Get the signature of your Activity Supervisor.
  5. Once signed, the paper copy of this form with signatures must be turned in to the Honors College front desk no later than 4pm the Friday of Finals Week.

Did you lose your one of your forms? If you lost any of your printed forms, you DO NOT need to create a new one. A copy of your original form can be found on the HARS website. Follow the instructions below:  

  • Agreement Form: To find a copy of your Agreement Form simply go to the HARS home page, scroll down to find a table list of all your Honors Activities, find the current semesters activity, click anywhere in the row, and a button labeled “Print Activity Agreement Form” will appear. This will allow you to print another copy of your form.
  • Completion Form: If you need to print another copy of your Completion Form simply go to the HARS home page, scroll down to find a table list of all your Honors Activities, find the current semesters activity, click anywhere in the row, and a button labeled “Print Activity Completion Form” will appear. This will allow you to print another copy of your form.

Honors College Capstone

Capstone Agreement Form Instructions

To be completed at the beginning of the semester in which you are starting work on your Capstone. You should also be registered for HON 322.  

  1. Enter the Honors Activity Reporting System (HARS). Find the link on the top of the Honors College website.
  2. On the left hand side of the page you will find a list of options for Honors Activity Forms. Select the link that says “Starting my capstone this semester”.
  3. You will be directed to the Capstone Agreement Form. Complete this form with all relevant information.
  4. Once you’ve submitted the Capstone Agreement Form you will be redirected to a page titled “Obtain Signatures”. A link for your Capstone Agreement Form will be available on this page and a copy will also be sent to your UIC email.
  5. You must print off the Capstone Agreement Form and get it signed by your Capstone Supervisor and Fellow.
  6. Once signed, the paper copy of this form with signatures must be turned in to the Honors College front desk no later than 4pm the third Friday of the semester.

Capstone Progress Report Form Instructions

To be completed at the end of the semester in which you submitted your Capstone Agreement Form.

  1. Enter the Honors Activity Reporting System (HARS). Find the link on the top of the Honors College website.
  2. On the home page you will find a table with a list of all Honors Activities you have submitted. Find the current semester and click once anywhere in the row.
  3. A button that says “Submit Progress Report” will appear below the row of the current activity. Select this button and complete the Progress Report.
    • You can add text to answer the questions on the Progress Report by utilizing the pencil tool that appears on the form. Please answer the appropriate questions before printing off the form for signatures.
  4. Get the signature of your Capstone Supervisor.
  5. Once signed, the paper copy of this form with signatures must be turned in to the Honors College front desk no later than 4pm the Friday of Finals Week.

Capstone Progress Update Form Instructions

To be completed at the beginning of the semester for which you are continuing work on your Capstone. You should also be registered for HON 322.

  1. Enter the Honors Activity Reporting System (HARS). Find the link on the top of the Honors College website.
  2. On the left hand side of the page you will find a list of options for Honors Activity Forms. Select the link that says “Continuing and/or completing my capstone this semester”.
  3. You will be directed to the Capstone Progress Update Form. Complete this form with all relevant information.
  4. Once you’ve submitted the Capstone Agreement Form you will be redirected to a page titled “Obtain Signatures”. A link for your Capstone Agreement Form will be available on this page and a copy will also be sent to your UIC email.
  5. You must print off the Capstone Progress Update Form and get it signed by your Capstone Supervisor and Fellow.
  6. Once signed, the paper copy of this form with signatures must be turned in to the Honors College front desk no later than 4pm the third Friday of the semester.

Capstone Completion Form Instructions

To be completed at the end of the semester in which you submitted your Capstone Progress Update. This form should only be submitted when your Capstone has been completed.

  1. Enter the Honors Activity Reporting System (HARS). Find the link on the top of the Honors College website.
  2. On the home page you will find a table with a list of all Honors Activities you have submitted. Find the current semester and click once anywhere in the row.
  3. A button that says “Submit Completion Form” will appear below the row of the current activity. Select this button and print the Capstone Completion Form.
  4. Get the signature of your Capstone Supervisor and Fellow.
  5. Once signed, the paper copy of this form with signatures must be turned in to the Honors College front desk no later than 4pm the last Friday of the semester (before Finals Week).
